** **full-time basis for 37.5 hours per week.

Playing a
** **key role in the Working Together Leighton Buzzard Collaboration Programme, which aims to improve health outcomes, promote, and enable se
_l_f-management for the Leighton Buzzard population. The role is part of a wider programme of collaboration across Central Bedfordshire.

Working with people, including the frail and elderly and those with long-term conditions, providing coordination of care and support. It is essential to fulfilling the role that you are able to attend the daily MDT meetings

Alongside the PCN team, you will be providing an all-encompassing approach to personalised care and enabling people to navigate through the health and care system.

Working closely with GPs, local authority, mental and physical community health services, and voluntary sector alongside practice teams, the core function will be managing a caseload of people, acting as a central point of contact, ensuring appropriate support is made available to them and their carers, supporting them to understand and managing their condition ensuring their changing needs are addressed, whilst working with a diverse range of people from different cultural and social backgrounds.

We are looking for a receptionist/care navigator to join our busy practice team and help make a positive impact on our local community.
- Excellent interpersonal and communication skills.
- Be organised, patient and empathetic. Ideally, experience of working in health, social care or other support roles including direct contact with people, families, or carers.
