Hours: 37 per week, working term time plus 3 weeks (42.2 working weeks per annum).

Actual salary (Subject to Review): £29,967.13 - £32,496.83 (Under 5 years’ service) and £30,642.62 - £33,229.34 (over 5 years’ service). With effect from 1 April 2023, the salary will increase to £30,102.23 - £32,643.34 (Under 5 years’ service) and £30,783.86 - £33,382.51 (over 5 years’ service).

The successful applicant will ensure that children with additional health needs are supported in their local schools as far as possible; that the schools make appropriate adjustments to support children’s learning and encourage school attendance in line with the school’s medical needs policy and accessibility plan. Co-ordinate the operation of the Multi-Agency Medical Panel (MAMP) including setting the calendar for, attending and participating in the MAMP and actioning outcomes. Under direction of the Multi-Agency Medical Panel (MAMP) to be responsible for co-coordinating and arranging support for children and young people with severe or long term medical/health needs (mental and physical), including those children who may be SEND with an additional medical/health need, beyond that which a school would be reasonably expected to provide. Maintain over site, in conjunction with the MERIT Pupil Referral Unit, of Stoke-on-Trent children and young people requiring education in hospital and in Tier 4 CAMHs settings both inside and outside of Stoke-on-Trent. To work with school governing bodies, trusts, school leaders, Educational Welfare Service, health and social care professionals, parents/carers to identify children who may be persistently or severely absent as a result of an additional medical/health need and ensure that the child’s additional medical/health needs are properly understood and effectively supported in order for the child to be able to access education. The post may include daily travel to various sites/schools across Stoke-on-Trent. You must be a skilled practitioner, who demonstrates a real enthusiasm for education and have significant experience of working with children and young people.
