The Home First Team has two new Transfer of Care Hubs one at BRI and another at Weston. The Home First team is made up of a number of organisations which consist of Acute hospital staff (Hospital Discharge Team), Local authority, social workers, Sirona, case managers, and voluntary sector staff.

We are looking for an innovative, inclusive leader who is passionate about delivering a service which is patient centred and ensures patients receive the right care at the right time in the right place. The post holder will work closely with clinical colleagues across the wider community, with Local Authorities, Community partners, Clinical Commissioning Groups and Voluntary Services, to support timely discharge for our patients.

You will require an enthusiasm for development and service improvement and you will work closely with the Operational Lead and Improvement Lead. You will be a key member of the Home First Management Team and therefore, will be required to deliver against targets and projects.

To line manage the Home First Team Leads and Homeless Support Team Lead at UHBW.

To provide day-to-day leadership of line-managed staff, such as sickness leave, performance, capacity and demand management.

Develop a standardised clinical approach across both sites, establishing cross-site clinical oversight, governance and quality assurance for the ADO.

To lead on the development of skills and knowledge across the Clinical team, supervise and mentor staff as required.

Horizon scan internally and externally, develop and maintain a network with other Trusts to support ADO and in developing a culture of continuous improvement.

Establish closer working relationships with community partners, ward teams and other acute hospitals

Supporting ADO to grow and develop a workforce to meet the needs of our patients.

To work as part of the Home First Management team assisting the ADO in developing strategic direction, new ways of working and project planning for the service to meet the Hospital Discharge and Community Support: Policy and Operating Model. To represent Home First Team at a senior level across UHBW.

Work cohesively with the operational lead to develop and maintain a high-performing service for our patients
