**Job overview**:
The Bodmin Community Assessment Treatment Unit (CATU) aims to undertake patient management nearer to home linking into the care provided by GP teams

Bodmin CATU is an exciting place to work; a nurse led unit at Bodmin Community Hospital. Created to accept patients from Home or ED, nurses and healthcare assistants are using their assessment skills and developing practical skills.

We are looking for dynamic people who wish to join our team and expand their skill set within support roles, with the opportunity to support dynamic patient flow within a multidisciplinary team which includes doctors, occupational therapists and physiotherapists.

**Main duties of the job**:
An exciting opportunity has arisen at Bodmin Community Hospital for a Progress Co-Ordinator to join the ward teams. You will play an important role in supporting the clinical staff in facilitating safe and timely discharge from hospital for patients.

You will help provide a seamless service for patients which will include cross boundary and collaborative working with the multi-disciplinary team along with colleagues in Adult Social Care and other outside agencies in the community setting.

This is an ideal opportunity for an enthusiastic and dynamic individual. You will need to have good interpersonal skills, IT skills and possess excellent communication and organisational skills.

We will offer a supportive working environment in which to continue to develop your skills, a structured in-service training programme and appraisal and supervision systems.

Bodmin Community Hospital is a busy community hospital with a variety of services including an MIU department, out-patient department, Xray and two wards - Bodmin CATU with 24 beds, Anchor Ward with 13 beds and the Woodfield Stroke Unit with 9 beds
