**Job Title: Health and Wellbeing Coach (Wirral)**

**Responsible to**:Health Junction & Primary Care Network (PCN) Leads

**Accountable to**:Health Junction &PCN Clinical Directors

**Location**:GP Practices within Wallasey PCN (Wirral)

**Contract Type**: One year fixed

**Hours**:Full Time, 35 hours per week

**Salary**: £26,125 per annum plus pension after qualifying period

**Holidays**: 5 weeks (25 days) holiday plus Bank Holidays

**Purpose of the role**
- As a Health and Wellbeing Coach you will be a key member of the multi-disciplinary team (MDT) working across, and based within, the GP practices that form Wallasey PCN. You will work with patients and groups holistically, considering the whole person.
- Health coaching revolves around using coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing. The role also covers support for community safety, individual patient case management, follow up and on-going patient support
- The post holder will have a key role in helping to raise the local population’s awareness of the support groups and opportunities available to assist them in achieving their health and wellbeing goals.

**Role Specific Key Tasks**
- Provide personalised support to people with long-term physical and mental health conditions to build knowledge, skills and confidence to live well with their health condition and become active participants in their own healthcare
- manage and prioritise a caseload in accordance with the health and wellbeing needs of the PCN population
- Coaching and motivating patients through multiple sessions to identify their needs, develop a personalised plan and set goals
- Supporting personal choice and positive risk-taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses
- Working alongside & in partnership with the existing Health & Wellbeing Coaches, Social Prescribing and Care Coordinators teams
- Developing referral pathways into existing services with health, social care, and community and voluntary sector providers and multidisciplinary teams (MDT)
- Where required and as appropriate, the Health and Wellbeing Coach will refer people back to other health professionals within the PCN
- Promote the service within the Primary Care Network, both for users and clinicians, building positive working relationships
- Work collaboratively with Public Health England supporting pop-up Health and Wellbeing Clinics and, when required, vaccination clinics
- Utilise existing IT to screen patients, with an aim to identify those that would benefit most from health coaching
- Actively collect and maintain data that will demonstrate patient improvement in key areas

**Data capture**
- Use EMIS templates to capture data and Key Performance Indicators (KPIs)
- Use Patient Reported Outcome Measures to capture key information to evidence the impact of Health Coaching on patient health and wellbeing
- Use the case management system to log the person’s progress
- Work closely with GP practices within the PCN to ensure that Health coaching referral codes are inputted to EMIS and that the person’s use of the NHS can be tracked, adhering to data protection legislation and data sharing agreements

**Professional development**
- Complete all mandatory training.
- Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
- Take part in regular supervision and reflective practice sessions to enable you to deal effectively with the difficult issues that people present.

**Miscellaneous**
- Work as part of the team to seek feedback, continually improve the service and contribute to business planning.
- Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.
- Duties may vary from time to time, without changing the general character of the post or the level of responsibility.

**Note**

**As you will be working within a Primary Care setting it is our expectation that all staff will have, or be prepared to have, the seasonal flu (& where appropriate) the COVID -19 vaccinations**

**Job Type**: Fixed term contract
Contract length: 12 months

Pay: £26,125.00 per year

- GCSE or equivalent (required)

**Experience**:

- Health and Social Care: 1 year (preferred)

Licence/Certification:

- Driving Licence (required)
