We are seeking a Community Matron to work within the Mid Beds Complex Care/Primary Care Home Team providing long term condition and minor illness assessments for patients that meet the service two-hour response criteria.
You will be a strong leader, focused on achieving real improvements in outcomes for patients and will be confident in your ability to support, coach and develop individuals and teams to deliver consistently high quality services. You will be innovative, forward thinking, self-driven with excellent communication and interpersonal skills. You will be an experienced Registered Nurse with top to toe assessment and independent prescribing skills with a background in minor illness or chronic disease management.
You will take pride in your work and be highly motivated and dynamic, have a flexible approach and be able to embrace change. Being a collaborative worker and the ability to network and build relationships with a diverse group of stakeholders is essential.
Due to the nature of the post the applicant must be able to meet the travel requirements of the post. Post is full time.
- The postholder will provide proactive case management for patients suffering with long-term conditions and/or minor illness within the care home or home environment.
- The postholder will support the 2-hour response requirement for any patient requiring a physical review or assessment within the community
- The postholder will enable and educate patients and their carers to help them live with and manage their disease and/or minor illness and discharge them within 60 days of referral into the service and follow agreed patient pathways.
