Palliative Care Associate Practioner

An exciting opportunity has arisen for a B4 Palliative Care Associate Practioner to join the HHFT Palliative Care team based in the community in Basingstoke.
The Role of the Palliative Care Associate Practitioner is to (assist and) support the qualified health professionals in ensuring optimal care for our patients.
The Palliative Care Associate Practioner will form an integral part of the multidisciplinary team approach in caring for the physical, social, and psychological needs of patients diagnosed with a life limiting illness and their carers.
The Palliative Care Associate Practioner will work closely with the Clinical Nurse Specialists (CNSs) and Palliative care Social Worker to provide a patient centred approach.
The post holder will work independently within the remit of the role but under the guidance and supervision of the Clinical Nurse Specialists or Social Worker.
- To support the team (qualified practioners) to provide a seven-day service to patients in the community with a life limiting illness
- Provide effective evidence based care for patients and their families referred to the service under the indirect supervision of qualified practioners
- Use personal effectiveness and knowledge to contribute to the development and delivery of a high quality, cost effective service.
- To work closely with the multidisciplinary team
- To promote a positive image of the Trust, service and department.
- Provide level 1 psychological support within own knowledge boundaries following appropriate training.
The Palliative Care business unit operates across all Trust sites. This includes the hospice in Andover and new hospice in Winchester. In Basingstoke we have a hospital and community team.
