**Flint House Police Rehabilitation **is a registered Charity offering rehabilitation services to ill or injured serving and retired police officers suffering from a wide variety of physical and mental health conditions. Every year we treat over 3,000 police officers. Located just outside Goring on Thames, Flint House runs a Mental Health Support Programme supporting serving police officers and staff who are experiencing low level common mental health issues, in a safe and relaxed environment in a beautiful countryside setting.

We currently have an exciting opportunity for a full-time **Mental Health Practitioner** to join our team. You will work as part of a small, close knit team of professionals to deliver the unique Flint House Mental Health programme in an efficient, effective and caring way in line with professional Codes of Practice and Flint House guidelines. Supporting serving police officers to self-regulate their thoughts, feelings and behaviours, and to explore coping skills, you will provide the highest standard of mental health rehabilitation services for each individual patient.

Hours of work: 40hrs per week, Monday to Friday.

**Principal Accountabilities**
- Assessing patient referral forms, contacting patients to clarify information given, booking telephone assessments where appropriate following initial identification of suitability for attendance in line with Flint House Inclusion / Exclusion criteria.
- Conducting telephone assessments using the telephone assessment template in order to confirm that the patients’ individual goals can be met by the programme and to ensure that the programme content can deliver what the patient needs and expects.
- Facilitating and managing the group dynamics within the daily psycho-educational sessions, educating and teaching patients regarding mental health issues whilst also encouraging proactive and healthy discussion.
- Evaluating the participation of individual patients within the group sessions in order to feedback all relevant information to the rest of the team to ensure the timely and appropriate identification of risk and continuity of patient care.
- Managing brief intervention 1:1 sessions with patients in a structured and supportive manner ensuring that discussions are skilfully managed and directed in line with patient needs, expectations and the time scales available.
- Assisting the Rehabilitation department on a rota basis to conduct the weekly group health walks in the local, mixed terrain countryside in order to promote the health and wellbeing of all Mental Health patients during their stay at Flint House.
- Identifying the specific and safe discharge needs of patients based on their individual requirements and risk assessment, providing recommendations to appropriate support services such as specialist agencies, GPs, charities, offering self-help material, further treatment for trauma and any workplace adjustments in order to ensure continuity of care.
- Actively contributing to departmental and post-discharge meetings in order to present individual patient outcome data that supports service evaluation, future treatment planning and for audit and quality purposes.
- Working as part of a multi-disciplinary team alongside all other departments within Flint House, to ensure holistic care and the comprehensive delivery of the total Mental Health programme.

**Profile of Job Holder**

**Essential**
- Mental Health trained professional with a current appropriate qualification e.g. RMN, OT.
- Previous experience of having carried out 1:1 assessments of patient needs and risk assessment.
- Experience of delivering psycho-educational group sessions and facilitating discussion through group work.
- Capable of completing walks of a nature and distance determined by the patients’ abilities and needs, but which would normally be of between 3 and 5 miles and to be completed within a 2 hour period.
- Own transport.

**Desirable**
- Experience of facilitating brief intervention 1:1 sessions.
- Recent demonstrable experience of having presented information to groups of individuals (max 12 people).
- Previous employment involved in delivering mental health care, ideally short term, working with patients with mild to moderate mental health needs.
- Demonstrable ability to quickly engage with patients in order to assess needs and implement appropriate care.
- Previous role(s) to include responsibility for patient discharge and liaising with all appropriate services.
- Competent in the use of MS Office suite together with previous experience of having used computerised medical note systems.
