**The main duties include**:
- Clinical session time is allocated specifically for MDT’s and Community case load reviews.
- To provide advice to colleagues across community service who touch on the care of elderly patients.
- Proactively engage with the multi-disciplinary team within Intermediate Care Services offering specialist advice and support as required.
- Proactively engage with the multi-disciplinary team within complex case managers offering specialist advice and support as required.
- Advisory Support (face to face, by telephone or video calls) will also be provided regarding patient care to colleagues. This will extend across other services including care homes.
- Support for the Frailty virtual ward on a daily basis.
Individual appointees should have been fully trained and accredited in all aspects of Elderly Care, with special interests in Frailty and Comprehensive Geriatric Assessment, which will support the continuing development of Frailty in Walsall Healthcare NHS Trust, Community Services division.
All applicants must have a full GMC registration with a licence to practise.
The Trust provides integrated acute and community Healthcare in the West Midlands serving a population of 260,000 residents. Walsall Manor Hospital houses the full range of district general hospital services. A new integrated critical care unit and a new neonatal unit have recently been built, and building is due to commence on a new Urgent Care Centre housing a new Emergency Department.
We provide high quality, friendly and effective community health services from multiple sites covering Walsall. Our multidisciplinary services include rapid response and homebased care, so that those with long term conditions and the frail elderly can remain in their own homes.

1. General
- To provide advice to colleagues across community service who touch on the care of elderly patients and frailty.
- Proactively engage with the multi-disciplinary team within Intermediate Care Services offering specialist advice and support as required
- Working alongside Complex Case Manager and Frailty mangers for patients being supported at home.
- Actively participate and support patient identified and managed on the frailty virtual ward.
- Advisory Support (face to face, by telephone or video calls) will also be provided regarding patient care to colleagues. This will extend across other services including care homes.
- Some clinical session time is allocated specifically for MDT’s and Community case load reviews.
2. Clinical Cover for Community Geriatrics and Frailty
- In case of your absence, you should ensure that other Consultant Medical Staff or Junior Staff are adequately informed and available to cover those clinical needs.
3. Junior Medical and Nursing Staff
- In conjunction with Consultant and Senior colleagues, to play a full part in the professional supervision and management of junior medical, nursing & AHP staff.
- In conjunction with Consultant and Senior colleagues, to take responsibility for and devote time to teaching, examination and accreditation duties as required for junior medical, nursing & AHP staff. This is an important role for any Community Consultant who has a duty to share their skills in dealing with complex ethical and medical decision making.
4. Clinical Governance and Clinical Developments
- In conjunction with Consultant and Senior colleagues, to take part in medical audit, quality improvement projects and research as appropriate.
- In conjunction with Consultant and Senior colleagues, to ensure that the requirements of clinical governance are met.
- In conjunction with Consultant and Senior colleagues to deliver educational training across Walsall Together Integrated Care Partnership.
5. Management & Service Development
- To contribute to innovation and design of community services alongside community division colleagues and the wider trust under the direct of the community medical lead.
- In conjunction with the Divisional Manager, Consultant & Senior colleagues, to play a full part in developing & implementing new ways of working which are in line with modernisation principles and fit for the future.
