Are you seeking a new opportunity to care for patients in their home environment as a Band 4 Assistant Practitioner/ Nursing Associate?

Do you have the passion and motivation to care for patients in a community setting?

An extensive induction programme is also offered to ensure staff build confidence and skill within the role.

As a Nursing Associate in the community, you will empower patients to self manage their health conditions whilst meeting the needs of the complex patient in the community and avoid admissions to the acute sector where appropriate.

The service operates 7 days a week from 0800-1830.

What we need from you

A qualified RMN with passion and drive for developing your clinical skills in a changeable environment

A keen interest in new and upcoming technologies within Nursing and Healthcare

Be self-motivated and able to work autonomously as well as showing initiative and promoting new ideas on how to improve the service

Be compassionate, caring and creative with excellent communication skills

A flexible and positive attitude with a willingness to accommodate our 24/7 service
