This is an opportunity to work as part of a charity committed to our Vision of enabling everyone in our communities to achieve better mental health. TOG Mind is a fast-paced charity delivering a wide variety of mental health and emotional wellbeing services. In your role as Access and Crisis Navigator, you will work as part of the access and crisis team and you will be responsible for:

- Receiving referrals from adult social care the various hospital teams and mental health wards to support individuals with their mental health
- Navigate clients within their community to live well after discharge from hospital
- Carry out initial discussions and various forms of support such as Supported Self-Help and wellbeing sessions
- Be based at Royal Oldham Hospital 1-2 days per week working closely with hospital staff

**Job Types**: Full-time, Fixed term contract
Contract length: 12 months

**Salary**: From £27,499.00 per year
