Oldham, United Kingdom Pennine Care NHS Foundation Trust Full time

The Crisis Care pathway for children & young people across Greater Manchester (GM) has been totally transformed. Four multi-agency, multi-disciplinary Rapid Response teams serving the whole of GM are at the heart of the transformation. These teams will provide crisis assessment and management and will offer intensive support post crisis for children, young people and families. The post holder will be based within one geographical cluster, however some cross-cover may still be provided across the full Greater Manchester foot print.

The Rapid Response Teams and the overall pathway emphasise a culture of accessible, high quality, holistic, child centred, recovery focused care, enabling you to effectively address the needs of children and families when they are at their most vulnerable. As such these posts will be enormously rewarding.

The new crisis care pathway enjoys a high degree of national & local recognition and interest. Therefore these are high profile posts offering you potential opportunities to conduct service based research and to present at conferences and other events. You will receive a structured induction and comprehensive training programme, along with a robust programme of continuous professional development.

As a member of the Rapid Response Team you will be at the heart of a well funded, well organised, hugely valued work-stream. You will deliver robust crisis assessment, effective risk management and intensive support to children, young people and families who are in or immediately post crisis. The main objective of the service is to prevent unnecessary and often unhelpful admissions to A&E, Paediatric inpatient settings and acute Inpatient admissions where clinically appropriate.

You will offer up to 72 hours of intensive follow up and develop longer term care plans to identify and address the holistic needs of young people and families across Greater Manchester. You will also link with specialist in-patient services, referring and co-managing admissions within a recovery model.

We are proud to provide high quality mental health and learning disability services, both inpatient and in the community across five boroughs of Greater Manchester - Bury, Oldham, Rochdale, Stockport and Tameside and Glossop.

Our vision is for a happier and more hopeful life for everyone in our communities and our staff work hard to deliver the very best care for the people who use our services. We’re really proud of our #PennineCarePeople and do everything we can to make sure we’re a great place to work.
- Be available to assess and manage young people presenting in crisis within a new Assessment and Reduction of Risk framework.
- Offer up to 72 hour intensive support and intervention, jointly with VCSEs and other agencies across all GM boroughs
- Work across traditional boundaries to develop holistic, recovery focused care plans to promote resilience and prevent relapse
- Refer to specialist in-patient services and co-manage admissions to reduce Length of Stay
- Ensure the accurate documentation of assessments and interventions.
- You will have a relevant core professional qualification in mental health or social care plus evidence of continued professional development in a CAMHS related area.
- You will have evidence of professional registration.
- You will have substantial experience of risk assessment and risk management.
- You will have substantial experience delivering brief, focused evidence based interventions such as CBT, Problem Solving, Family Interventions, Parenting Interventions and skills coaching.
- You will be highly skilled in understanding and managing complex presentations within complex systems.
- You will have a high degree of emotional resilience enabling you to perform at a high level and communicate empathically and effectively even when people are highly dysregulated and potentially abusive.
- You will have a genuine and passionate belief in the child centred, needs led, recovery focused values base of the service.
- You will need to recognise the value of flexibility and availability for young people and be able to work weekends, unsocial hours and have use of a car or suitable means of transport to enable support of young people across GM.
