The Practitioner will provide comprehensive mental health service to adults of working age who experience moderate to severe mental health problems in a community setting. -Utilise the Care Programme Approach to support and promote the recovery of service-users under the care of the service. -Working collaboratively across a range of disciplines to continuously promote a needs based, recovery focused approach to all service-users. -Work within, and promote, an integrated approach to service delivery.
- Develop clear relapse recognition/prevention plans in a collaborative manner and share with MDT, service-users and carers where appropriate. -Develop meaningful engagement with service-users and carers. -Offer support as needed as part of Duty Rota. -The Team offer a Monday-Friday service within the hours of 9am-5pm.
- Flexible working can be discussed in line with Trust Policy. Person Specification; Qualifications; Essential; -Registered Mental Health Nurse or allied health professional (Occupational Therapist/Social Worker). -Evidence of continued professional development. Desirable; -Short courses or experience up to post-graduate diploma level.
- Mentorship qualification (or equivalent) or willingness to compete training -Psychosocial/CBT Qualification or experience in delivering these interventions. -Non-medical Prescriber EXPERIENCE; Essential; -Experience of working within a Mental Health setting as a qualified member of staff -Experience of developing collaborative care plans and risk assessments -Experience of working within a Multi-Disciplinary Team -Experience of working with people with a range of complex mental health problems Desirable; -Experience of working within a Community Mental Health Setting -Experience of clinical supervision of other staff KNOWLEDGE; Essential; -Knowledge of relevant legislation underpinning role; Mental Health Act, Care Act, Mental Capacity Act -Understanding of working within an Integrated Team -Comprehensive understanding of Safeguarding adults procedures and practice. -Awareness and understanding of Care Programme Approach -Working knowledge of therapeutic interventions and assessments for people with complex mental health needs -Awareness of key national and local policies and drivers in relation to adult mental health services Desirable; -Psychosocial Interventions -Knowledge of CCG and Local Authority commissioning processes SKILLS AND ABILITIES Essential -Ability to undertake the role of a Care Co-ordinator -Evidence of ability to provide specialist mental health assessment and management/assessment of risk and demonstrate sound clinical judgement. -Excellent written and interpersonal communication skills.
- Ability to manage own emotions and deal with others in stressful situations. -Ability to engage with, and provided therapeutic interventions too, client group and carers. -Ability to prepare, record and complete relevant reports to a professional standard. -Ability to manage a diverse and complex caseload and prioritise own time and work under pressure.
- Demonstrating and valuing diversity and respond constructively to discriminatory behaviour. -Ability to use I.T systems efficiently. Work Related Circumstances Essential -Use of a vehicle or access to means of mobility to travel across Trust footprint in line with service needs. -Willing to carry out all duties and responsibilities of post in accordance with the Trusts Equal Opportunities and Diversity policies.
