Job Responsibilities; -The Practitioner will provide comprehensive mental health service to adults of working age who experience moderate to severe mental health problems in a community setting. -Utilise the Care Programme Approach to support and promote the recovery of service-users under the care of the service. -Working collaboratively across a range of disciplines to continuously promote a needs based, recovery focused approach to all service-users. -Work within, and promote, an integrated approach to service delivery.
- Develop clear relapse recognition/prevention plans in a collaborative manner and share with MDT, service-users and carers where appropriate. -Develop meaningful engagement with service-users and carers. -Offer support as needed as part of Duty Rota. -The Team offer a Monday-Friday service within the hours of 9am-5pm.
- Flexible working can be discussed in line with Trust Policy.
