This Macmillan supported role will focus on the living with and beyond cancer agenda and the survivorship programme established within the long term plan. The post is pivotal in supporting the existing team and allied health professionals within the group whilst contributing to a multi professional vision. Support co-ordination during the patients cancer journey specifically related to their handover from the Urology directorate and community care colleagues To be professionally accountable for the delivery of standards of professional practice and behaviours as set by the Nursing & Midwifery Council. Demonstrate the impact of Macmillan Right By You Principles by, but not limited to, regularly recording activity as required to monitor the applicable Performance Metrics To deliver compassionate person-centred care, and treat patients, service users, their families and their carers with consideration, dignity and the respect you would want for yourself or your family.
