The Short-Term Services team are recruiting a full time permanent Clinical Team Leader to join our team. Your job is a key health and social care role located in the Moor to Sea integrated service unit.

Your purpose is to work with adults with care and support needs to identify and deliver personalised, good quality outcomes to the highest standards to support and maximise their independence and well-being. The job will integrate elements of both therapy and social care to ensure holistic assessment.

As a Clinical Team Leader you will be working as a trusted assessor, assessing functional ability, providing advice, daily living equipment, assistive technology and digital care solutions while promoting and maximising independence and well-being.

You will work as part of health and social care teams, removing organisational boundaries and facilitating the whole pathway within the designated clinical area. You will be the pinnacle of joined up effective integration between multiple organisations, ensuring the adult is central. The role will be defining training programmes across health and social care and will lead the way in developing this for other health and social care teams.
- To effectively lead and supervise support workers in designated teams including the recruitment and selection process
- To manage a defined caseload and maintain own records as an autonomous practitioner under the management of a qualified senior therapist, using evidence based adult centred principles to assess, plan, implement and evaluate interventions in designated clinical areas. Showing evidence of high levels problem solving, decision making and independent judgement in a variety of community settings and situation

Social Care Teignbridge is part of short-term services that are provided to people in their own home. Short term services include Reablement and Rapid Response. These services are provided by Torbay and South Devon NHS foundation trust which is an integrated organisation providing both health and social care in South Devon. Services are co-funded by the NHS and Devon County Council.

There are four team providing reablement and rapid response services across Torbay and South Devon. We work very closely to support each other, ensuring we deliver a first-class service.

Primarily you will be based in Newton Abbot providing services out of Estuary House. We operate a hybrid way of working. There will be a mixture of office and home-based working.

The service operates 7 days a week and some weekends and bank holidays may form part of your shift pattern. A normally shift pattern is Monday to Friday 09:00-17:00.
