**Rate of pay**: £19,836 per annum

**Hours**: 37 hours per week

**About the role**

As a Community Liaison Worker you will support people in the community to be able to remain as independent as possible in their own home and reduce the risk of crisis support. You will develop support to enable them to manage their long-term health conditions and enable them to feel less isolated. By helping your clients regain their confidence you will be improving their general wellbeing and quality of life.

You will take referrals from other professionals and members of the public and undertake a holistic assessment of clients in order to help them maximise their independence. This is a varied role where you can use your own initiative to get the best for your clients. You may be arranging a house clearance and cleaning one day, organising emergency food parcels and prescription collection another. By supporting people with their housing issues, finances, medication and follow up appointments, you will help them have more control over their own life.

The role involves visiting clients in their own homes to monitor the service as well as making telephone check calls to offer support.

**About you**

You must be a car driver with access to your own vehicle. If you are motivated by challenge and enjoy seeing problems through to their solution, this is the role for you.
