To have responsibility for facilitating, enabling and supporting interdisciplinary decision making in order to achieve optimal patient flows and utilisation of available resources across all relevant hospital sites. To actively manage patient flows by maintaining processes that safely expedites patient placement and discharge. The jobholder is responsible for the timely and appropriate placement of patients requiring admission to the Trusts bed base. 3.

JOB DIMENSIONS The job holder will work alongside a band 6 sister/ charge nurse each shift. The job holder needs to work efficiently to facilitate the quick movement of patients from the emergency department into the medical bed base. The job holder will need to be efficient in multitask management. The job holder needs to work well in high pressured situations and be confident to communicate with all levels of the multidisciplinary team.

The job holder needs to be a dynamic individual that is eager to learn during this developing job role. The job holder will help to facilitate Patient discharge from the emergency department to home, alternative external health or social care provision, or transfer to the appropriate in-patient clinical service. 4. KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED Qualifications Diploma level education or equivalent demonstrable experience Experience Acute Health Care experience Experience with patient flow or process management Experience of urgent or emergency care in acute hospitals Able to communicate with staff at all levels in the organisation Skills Influencing and Negotiation. Excellent written and verbal communication skills. Organising and prioritising workloads Time management Team working. Problem solving Experienced in the use of IT systems Knowledge Current issues in Healthcare.

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY Develop effective working arrangements within the Emergency Department (ED) that allows the timely and appropriate placement of acute admissions.

Facilitate early discharge of patients from the ED. Actively follow the Departmental Escalation Plan and record electronic updates and status reports as per guidelines. Take a lead role in the creation of capacity to meet demand and liaise with Clinicians and Matrons when insufficient demand is created. Participate in systems and processes that maintain and improve patient flows.

Develop effective partnerships with clinical and clerical staff and an environment which fosters open communication and participation. Participate in developing
