Support the LD& Autism Professional Lead to ensure that the care of patients with a LD and /or autism including those who transition from child to adult services is appropriately managed and delivered.
- Participate in the co-ordination and planning of care for people with Learning disabilities and autism, working in partnership patients/carers, other health providers and partner agencies.
- Assist patients/carers to make choices throughout their care journey and advocate for their needs.
- Ensure the principles of the Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards (DOLS) are incorporated into the care plans of people with learning disabilities and autism.
- Improve the experience of care of people with Learning Disabilities when receiving care at STHFT.
- Work with clinical staff to implement best practice identified through research, national and local guidance.
- Ensure patient feedback and the service user voice is incorporated into service improvements and re design.
- Develop resources to support staff working with young adults with learning disabilities and/or autism.
- Identify and meet the learning and development needs of healthcare staff in relation to their knowledge of providing services to people with learning disabilities and/or autism.
- Evidence improved outcomes for young people with Learning Disabilities and/or autism receiving care at STHFT

**The post holder will**:

- Be a key member of the Care of Children and Young People Steering Group and other relevant internal groups and meetings
- Work with a degree of autonomy across the five units of the Trust.
- Provide a point of contact for internal and external stakeholders requesting information about patients with a learning disability
- Support the LD Professional Lead to interpret and implement national policies and guidelines on transitions issues to meet the needs of patients and contribute to the development of policies and procedures.
- Support learning and development opportunities between partner agencies and community groups
- Contribute to reports and briefing papers as required for appropriate committees, and assist the Head of Safeguarding, Professional Lead and Nurse Director for LD & Autism in formulating an Annual Report.
- Actively seek the views of adults, children and young people and their carers, using appropriate and accessible methods of engagement. Ensure views are fed into service improvements developments and service re
- design.
- Where the post-holder continues in a clinical role, all duties and responsibilities in respect of quality professional practice should be met and maintained in line with specific clinical role, Trust policy and procedure and professional background.
- Advise, inform and escalate safeguarding concerns as appropriate
