An exciting opportunity has arisen for an individual to join our team as a Learning Disability and Autism Professional Lead.

The post holder will work in accordance with the STHFT Learning Disability and Autism Strategy and contribute towards achieving the Trust’s Objectives. Participate in the co-ordination of health services at STHFT for people with Learning disabilities and autism, working in partnership patients/carers, other health providers and partner agencies to improve their health outcomes.

Other duties will be to assist patients/carers to make choices about their care and treatment and advocate for their needs whilst accessing services at STHFT. Ensure the principles of the Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards (DOLS) are incorporated into the care plans of people with learning disabilities and autism.
- Ensure that people with learning disabilities and/or Autism at STHFT have full and appropriate access to healthcare within the hospital setting
- Liaise with all relevant parties to obtain information and plan effectively for all hospital visits and to ensure effective discharge planning
- Establish a high-profile presence within STHFT and act as an expert & resource in the field of learning disabilities and/or Autism
- Provide expert advice to staff for the Mental Capacity Act and Best Interest decision making processes for people who may lack capacity to make specific decisions
- Lead, contribute to & facilitate the LD and autism agenda with the Head of Safeguarding, to include strategy setting and objectives
- Establish & facilitate a programme of training, coaching and mentorship to nurses and healthcare professionals working within STHFT
- Jointly work with the Transition Team to support young people with a LD transitioning to adult services at STHFT
- Act as advocate for people with a LD
