Monday - Friday 9-5pm (can be flexible between the hours of 8am - 5pm)

**What you’ll be responsible for**:

- To provide skilled mental health nursing care to people in the community.
- To assume the role of care coordinator for a caseload.
- To work as a member of a multidisciplinary team.
- To have a theoretical and working knowledge of the Safeguarding Adults and Children's framework.
- To develop, implement and evaluate holistic programmes of care, including the assessment of carer's needs and assessments.
- To carry out clinical and therapeutic interventions, having regard to both legal requirements and Trust/National guidelines
- To make a positive contribution to the development of-mental health and social care services in Trafford.
- To maintain close links with other statutory and non-statutory professionals and agencies, and to be able to access resources from provider services, and to maintain good channels of communication.
- To undertake the role of care coordinator in accordance with the philosophy of the Care Programme Approach and associated Mental Health Act (1983) legislation.
- To provide support to the office Duty System as required, and be able to respond to emergencies (some aspects of which, under the guidance of senior staff).
- To work with people who are on the Assertive Outreach care pathway (under the supervision of senior members of staff).
- Act in accordance with practice placement standards for student nurses, liaising with link tutors from the University in the evaluation and validation of students.
- To organise and run community based depot and Clozapine clinics; playing a central role in the requisitioning or ordering of necessary equipment and medications, linking in with relevant pharmacy departments.
- Have an understanding of the Local Authority Commissioning process and how to access resources.
- Have the ability to review packages of care within the Care Act eligibility criteria to ensure that service users are eligible and that the package meets their eligible needs.
- To promote recovery through planned reductions in funded care as appropriate
- To undertake any other reasonable duty which is appropriate to the band, when requested to do so by Senior Staff.
- To be familiar with and comply with all Trust and departmental policies, procedures, protocols and guidelines.

The role will involve, but not be limited to you performing the above.

**You’ll have the following qualification/experience**:
**Qualifications**:

- Current professional registration with appropriate body in healthcare or social work
- Evidence of Continuing Professional Development
- Leadership or management qualification
- Post-graduate qualification in a relevant area
- Certificate in teaching and assessing in clinical practice as appropriate to own profession
- Clinical Supervisor
- Recognised sign-off mentor (or equivalent) for students or must be achieved within agreed timescale
- Key skills in literacy and numeracy level 2 (or equivalent)

**Experience**:

- Significant experience working with people with mental ill health in a community setting
- Working collaboratively with service users and their families/ carers
- Providing clinical supervision
- Mentoring student
