**Job description**

Optalis is the adult social care delivery partner on behalf of Wokingham Borough Council and the Royal Borough of Windsor & Maidenhead Council. Optalis is owned and funded by the 2 Councils. Optalis is not a private provider or an employment agency.

This is a multi-disciplinary team providing short term, intensive rehabilitation and end of life support for up to six weeks in a care at home environment.
The team comprises of occupational therapists, physiotherapists and rehabilitation care assistants working in the community to support residents to achieve or regain independence.
The service will assist to facilitate earlier discharge from hospital and prevent hospital admissions.
The service works closely with health partners to ensure the best outcomes for the service user.

To take, triage and process referrals into the service, working Monday to Sunday on a rota basis with alternate weekend working.
To manage and monitor a dispersed workforce outside of office hours, weekends and bank holidays included.
To be prepared to undertake a practical role to assist with the delivery of service when required.
To allocate and co-ordinate the work schedules of Rehabilitation Care Assistants working in the community with service users.
To supervise, appraise and support a group of Rehabilitation Care Assistants (RCA)who are community based.
To carry out initial assessment visits, complete risk assessments in the service users home, and agree a person centred support plan to enable rehabilitation goals to be achieved.

This role **is** **exempt from **the Rehabilitation of Offenders Act 1974. The DBS clearance level required is** **Enhanced with Adult Barred list**.
