We are seeking full-time and part-time Support Workers to work in our supported living setting, located in Hayes. Care workers also need to be able to work on the weekends as we have different shift patterns - Live-in and sleepover.

**Responsibilities**:

- Provide support and assistance to individuals with disabilities or special needs in line with their care plan.
- Assist with daily living activities, such as personal hygiene, meal preparation, and medication management
- Assist and encourage people to engage in everyday home, social, leisure and work activities, according to the individual’s choice.
- Support people with their health needs as required, e.g. taking medication and attending doctor’s appointments.
- Document and report any changes in behaviour or health status to the appropriate supervisor
- Collaborate with a team of professionals to create and implement individualized care plans.
- Be able to provide excellent customer service
- Knowledge and experience of CQC standards and CQC Compliance
- Undertake any other duties as appropriate.
- All staff are required to follow the Hygiene Code and all company policies and procedures related to it and the Health Act 2006. Failure to comply with any of these may result in disciplinary action up to and including dismissal.

**Requirements**:

- High school diploma or equivalent
- Minimum 3-4 months experience working with individuals with disabilities or special needs
- Enhanced DBS
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Patience, empathy, and compassion for individuals with diverse needs
- Ability to follow instructions and adhere to safety protocols
- Flexibility to work various shifts, including evenings, weekends, and holidays

**Pay rate**:
Live-in: £114.20

Sleepover: £70
