Band 6 Nurse Care Coordinator

Our team’s core hours are Monday - Friday 9am - 5pm. The services are in the middle of a service transformation, which is an exciting time, as we are exploring how we can further develop the service we provide, meeting the needs of our local community. This team is part of the Mental Health Transformation of services and our new blended team where our voluntary service colleagues are fully embedded within the team. This is definitely an exciting time to join this expanding, developing and modern service.

We also operate a Duty clinician system responding to those who require support in a crisis. Our team provides assessment, care planning, intervention and evaluation of those with a moderate to severe mental health diagnosis for ages between 18 to 65 (up to 70 if already open to the team). Our service focuses on developing and utilising your specialist mental health skills and embracing your core competencies as a Registered Nurse. Working within our team you will be involved in the planning of new services.

The post holder will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the operational policy. The post holder will work as an autonomous practitioner within an integrated team. This will involve the assessment planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the client. The post holder will be required to administer prescribed medication which includes intramuscular injections.

The team is made up of a rich resource of multidisciplinary professionals who support psychological ways of working. The team includes Registered Nurses, Social Workers, Senior Social Worker Practitioner, Peer Support Worker, Doctors, Psychologists, Assistant Psychologist, Occupational Therapist, Community Support Workers, Psychotherapists, Administrators and an Employment specialist as well as working closely within a designated locality with Primary care mental health link workers, the Local Authorities and voluntary services. Our team has recently seen developments with its restructure of new experienced Consultant Psychiatrists who have brought with them a wealth of specialist knowledge and experiences.

There are many opportunities, such as to participate in teaching at the recovery college, facilitate DBT, mindfulness, to join the AMHP training, to be involved in other groups and working closely in partnership with the drug and alcohol, CAMH and other services. In addition to the QI project above, there is opportunity to be part of other initiatives, such as our pilot of WhatsApp Business.
