Specialist Practitioner of Transfusion

To perform as a Specialist Practitioner of Transfusion (SPoT) assisting in the management of the service. To assist the laboratory manager in the implementation and delivery of a cohesive service, in conjunction with the clinical lead for the department and collaboratively identify areas for practice development. Understand and promote the requirements for Better Blood Transfusion, Safer Practice Notices, and other blood and blood component transfusion guidelines and regulations; to assess current practice and implement change where necessary to meet these standards and act as a role model of excellence in providing professional care. To educate, support and provide expert advice to clinical staff on transfusion guidelines and appropriate, safe transfusion practice promoting effective safe use of blood and blood components.
Where required, to assist healthcare professionals to develop appropriate plans of care for individual patients on all pertinent aspects of blood and blood component transfusion matters. To lead the development of training programmes for all clinical staff involved in the transfusion process. To manage the delivery of training as per the clinical training policy, ensuring tutorials, one to one training and competency assessments are fulfilled in accordance with the national guidelines and local objective requirement. To manage, investigate and provide action plans to resolve nonconformities that are recorded by internal and/or external audits, incidents or complaints to ensure the completion of corrective and preventative actions that have been identified.
This includes, but not limited to, investigating transfusion incidents and reactions in accordance with national and local policies. To ensure on-going accuracy of the Trusts Blood Transfusion Policy, both in terms of clinical practice and regulatory requirements. Where delegated, to be responsible for the formulation and implementation of protocols and policies for the department in collaboration with relevant consultants and laboratory managers. Promote and support effective communication systems between the clinical staff and the blood bank laboratory.
To assist in the dissemination of information, arising from the laboratory meetings, to the clinical staff through the communication structures in place and vice versa. To manage the day-to-day operation of the SPoT duties within the teams. This will include ensuring monitoring and review of trauma and transfusion cases, complaints, performance monitoring and actions are completed and recorded in a timely manner. In the absence of the Blood Sciences Laboratory Manager ensure that there is a continuation of key services when short term staff absences occur within the SPoT team, including when the absence will affect out of hours service provision where appropriate.
To manage the quality aspects of the service ensuring that product requests are met according to the agreed turnaround times and within the policies. To ensure the completion and reporting of SHOT, SABRE and MHRA reports, troubleshooting for the cause of any problems and actions required as a result of poor performance such as consistent bias or other quality problem. To lead, conduct or complete the audit (whether vertical, horizontal or examination) as required by local and national guideline/regulations of transfusion practice and to report audit findings to the Transfusion committee to ensure patient safety and to prevent or minimise litigation. To undertake National Comparative Audits in Blood Transfusion, as agreed by the Transfusion committee, to allow benchmarking against similar Trusts.
To work with the Blood Bank Laboratory to maintain complete cold chain and traceability for all blood products. To assist in the Risk Management Policy for the transfusion ensuring Risk Assessments are conducted, reporting of incidents, accidents and adverse events via the reporting structures that exist including the clinical lead and/or the pathology managers as required. To regularly assess the service performance providing information required for the quality monitoring report for the department. To assist in the cost effective use of blood components and ensuring participation in related audits of practice in relation to blood and component use.
To represent the department at local, regional and national meetings and conferences where appropriate. Where requested to represent the department in stakeholder meetings, decision-making and policy implementation. To participate as a key member of the Trust Transfusion Committee and Hospital Transfusion Team. To assist with the coordination of the Hospital Transfusion Committee and provide administrative assistance as agreed.
