There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: a. implementing and maintaining an effective prescription ordering process which includes the setup of repeat dispensing, issuing acute and repeat medication where indicated and supporting patients with medication queries b. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately c.

Signpost patients to the correct service g. Initiating contact with and responding to, requests from patients, team members and external agencies h. Read code data on Systmone i. Input data into the patients healthcare records as necessary j.

Manage all queries as necessary in an efficient manner k. Carry out system searches as requested l. Maintain a clean, tidy, effective working area at all times m. Support all clinical staff with general tasks as requested Secondary Responsibilities In addition to the primary responsibilities, the Care Coordinator may be requested to carry out the below IF support is needed from the teams within the practice: a.

Partake in an audit as directed by the audit lead b. Support administrative staff, providing cover during staff absences c. Scanning of patient related documentation and attaching scanned documents to patients healthcare records d. Complete opening and closing procedures in accordance with the duty rota e.

Ordering and monitoring of stationery supplies f. Management of surgery rota templates. g. Photocopy documentation as required h.

Data entry of new and temporary registrations and relevant patient information as required
