Step-Up virtual ward will be starting soon This is an ACP led, MDT model which provides additional clinical, and technology enabled support to a complex caseload of patients in their own homes, who would otherwise be admitted to an acute hospital, or supporting patients being discharged. Assess, diagnose, and treat patients at home, initiate wrap-round MDT support as required and lead on discharge from the service when safe and appropriate to do so.

You will make initial and follow up assessments of patients both in stable and unstable phases of their disease. Implementation of diagnostic skills including full history, appropriate clinical examination and request of investigations where required.

Provision of effective, efficient and appropriate high-quality care acting at all times in accordance with Codes of Conduct and to the stated level of competence and agreed protocols.

Working closely with EEAST (East of England Ambulance Service) we provide appropriately triaged category 3, 4 and 5 ambulance calls and provide admission avoidance service to patients in their own home who do not require ambulance attendance or hospital admission, and initiate appropriate care/treatment plan.

You will contribute to developing and progressing the care provided by relevant services by looking at alternative ways of delivering care, alternative pathways for patients and developing the ACP model to provide the care required by the Norfolk & Waveney population and the ICS.
