North Norfolk and Norwich CRHT provides 24/7 assessments and interventions as an alternative to hospital admission. You will be responsible for assessments, risk management, care planning, and co-ordination of care delivery within a well-resourced team, supported with regular supervision and providing opportunities to work in partnership with health and social care, linking into the wider multi-disciplinary service.

Main duties of the job

We are openly welcoming people from a diverse clinical background with experience in different areas of mental health.

Your role within CRHT will include:

Caseload management, supporting with the provision of mental health care services Supporting and advising other health care professionals within the wider team Giving advice about risk and demonstrating comprehensive assessment skills A positive role model and mentor to colleagues and junior members of staff Providing appropriate interventions to patients with mental illness (incorporating NICE guidelines) Initiate, create and maintain therapeutic relationships with patients and carers, ensuring that practice is patient centred. Supporting and develop the principles of joint partnership working and to be pro-acting in fostering positive interagency relationships and information sharing
