Health Care Support Worker Children's Diabetes

To assist the Children’s Diabetes Nursing Teams in the provision of direct clinical care to ensure an equitable service for children and families within the United Lincolnshire Hospital Trust catchment area
To support the development of the United Lincolnshire Hospitals Trust Children’s Diabetes service
To attend the complications due to excessive weight (CEW) clinic, once a month, at either Boston or Lincoln, to support the specialist nurses from the tertiary centre
Undertake independent home visits to deliver supplies, collect HbA1c samples, and support families and to carry out clinical care under indirect and direct supervision of one of the registered nurses.
Ensure appropriate prioritisation of work, in collaboration with nurses.
Assist with distraction for procedures and chaperoning as necessary.
Assist with clinical care of Children and Young People (CYP), in educational settings.
Assist in Diabetes clinic settings within the Trust.

Use a variety of IT systems, needed for downloads and collating data. Use of Trust IT systems, and some knowledge of Word, Excel.
Develop knowledge of TWINKLE and assist in inputting data onto it.
Assist in maintaining and updating the caseload database.
Attend countywide diabetes MDT meetings
Participate and help run any structured education groups.
Assist in organising / attending any away days / CYPs activity days
Make telephone calls and take messages as required.
Following investment into our paediatric diabetes service we are excited to be in a position to build a multidisciplinary team that will meet the needs of diabetic children in Lincolnshire and enables us to deliver Best Practice pathways.
