**Support Worker Roles & Goals**

Reporting to - Team Leader

Deputy Manager

Registered Manager

**Overall objective**

The Care Service involves a programme of personal care related tasks and household care management that is personalised for each Service User in the form of a care plan. Care duties will therefore include assisting the Service Users with a variety of duties and in doing so will respect the Service Users dignity, privacy, independence as far as is practicable.

**Specific objectives**

1. Support service users to attain and maintain safety, good standards of hygiene and personal care, ensuring assistance as appropriate

2. Support Service Users with mobility problems and other physical disabilities including incontinence and utilise equipment safely.

3. Support Service Users to maintain community integration and social stimulation.

4. Support Service Users to maintain their independence in all areas outlined within their individual plan of care

5. Ensure adequate care related records are maintained to a high standard.

6. Support service users in making choices that ensures a healthy, culturally appropriate and nutritious diet.

7. Contribute to the overall cleanliness of the home and involving the service users in these tasks as appropriate. Ensures all members of staff maintain the principles of Active Support throughout their interactions.

8. Demonstrates vigilance to the health and safety of service users, visitors and colleagues. This includes adherence to Company, Local Authority and National guidance for the Safeguarding of people who use our service.

9. Conform to all organisational and home Policies and Procedures

10. Carry out accurately and in a competent manner, instructions from Managers and supervisors, adhering to individual care plans relating to Service Users

11. Actively talk to and listen to Service Users, allowing for their personal choice in the home

12. Report any accident / incident that may occur to line manager.

13. Perform other such duties that may be reasonably required

14. Make yourself available on a regular basis at an agreed, appointed time to assess and review your personal and professional development and progress which may be by way of supervision, appraisal.
