- Windsor Ascot & Maidenhead (WAM) Community Mental Health Team (CMHT) is a multi-disciplinary team providing services within Windsor Ascot & Maidenhead for the local residents who require secondary mental health services.- We operate an integrated service with the NHS and WAM local Authority which aims to provide a comprehensive range of interventions for service users with enduring and severe mental illness.**Objectives of the Service**
- To provide biological, psychological and social care based treatment and enable service users to make recovery from their mental disorders.- To achieve timely and adequate services by the provision of comprehensive assessment under the Care Act 2014.- To provide a person-centred care using the care programme approach (CPA).- To offer carer’s assessment and identify their needs for support.- To adopt a holistic approach and promote service users’ mental and physical health well-being.- To create a manageable risk and work towards suicide prevention.- To promote equality and challenge stigma and discrimination.- To fulfil statutory duty in accordance with relevant legislations, regulations and code of practice.- To promote and implement multi agency working.- To provide care co-ordination and work on relapse prevention plan and prevent hospital admission.- For a further discussion about the post please contact Jessica Martin, AMHP Lead on 01189 047 300.- Optalis is committed to safeguarding and promoting the welfare of our staff and customers.
