Substance Misuse and Mental Health Practitioner

An exciting opportunity has arisen for dedicated and enthusiastic Substance Misuse Practitioners to join our growing team of multidisciplinary clinicians providing care to patients within Nottingham and Nottinghamshire. These are pivotal roles working across specialist mental health services and substance misuse provider services.
In Nottinghamshire there are 3 Integrated Care Partnerships (ICP) which cover Mid Nottinghamshire, South Nottinghamshire and Nottingham City. The Substance Misuse Practitioner roles will work across teams/services within a dedicated ICP to offer high quality interventions to service users and advice, training and consultation to colleagues working within the locality.
These roles will work across the ICP focusing on enhancing substance misuse capability within adult mental health services and improving the delivery of evidence-based care for service users with co-existing substance misuse/mental health. In addition the post holder will provide mental health specialism within partner substance misuse services.
The post holder will work with colleagues of all disciplines to develop knowledge, skills and confidence in working with people with substance misuse, ensuring that it remains a core aspect of the work of all clinicians.
These posts will work closely with the Local Mental Health Teams across adult services as part of an all-age adult pathway and also working into Substance Misuse provider services in the local area.
To provide expertise and contribute to substance misuse being systematically identified and responded to in a timely, evidence-based manner
To implement systems to ensure all service users are being assessed for alcohol and drug use as part of a comprehensive assessment and information is recorded in line with Trust standards
To provide specialist assessment and clinical formulation to clients with complex mental health needs and co-occurring substance misuse.
To provide expertise and contribute to development of effective care plans to support patients with co-existing substance misuse/mental health
Forge strong links with partner substance misuse services - it is expected that the post holder will spend a proportion of their time working within partner agencies to deliver training, advice and consultation in relation to mental health.
The post holder will act as a bridge between LMHTs and partner agencies to identify and support patients who may need transfer of care between services and provide support to facilitate this transfer.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
