The post holder will be expected to support the BHR IDH team, service and organisation to achieve the Trust’s Values in their day-to-day work. These are the 5P’s:

The post holder will achieve this by working as part of the BHR IDH based in Goodmayes Hospital to provide high quality, confidential and comprehensive administrative service.
The discharge to assess model has been implemented since March 2020 with an intention to support more people to be discharged to their own home. The expectation of the service is to reduce the length of stay for people in acute care, to improve people’s outcomes following a period of rehabilitation and recovery and minimise the need for long-term care at the end of a person’s rehabilitation.
BHR IDH is a 7-day, Monday to Sunday 08:00 AM to 08:00 PM service. Staff will be working on a rota basis including weekends and bank holidays.
- To be responsible for organising and prioritising own delegated workload
- Work as part of the multidisciplinary team to provide care to patients in the community setting by carrying out and monitoring treatment programmes as directed.
- To work under the indirect supervision of and alongside Occupational Therapists and Physiotherapists to carry out assessments.
- To liaise with the MDT, internal and external agencies including voluntary and social services, patients, relatives and carers.
- To be responsible for competent use and maintenance of all relevant equipment, to report defects in equipment to the lead therapist and follow up
- To maintain stocks of equipment
- To assist with non-clinical induction of assistants and students as required.
- To collate and provide statistical information e.g. patient activity
- Analyse basic patient information to make informed decisions.
- Co-ordinate patient appointments.
- Organise home visits as directed, including liaison with patients, carers and other services/stakeholders.
- Monitor patient’s progress, taking account of their environment and report back to lead therapist so that clinical interventions can be adjusted accordingly.
- To be able to make accurate and effective records of activities in relation to the patients.

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High-Cost Area Supplement - London

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436).
