An exciting opportunity has arisen to join Warrington Borough Council’s Intermediate Tier Services, in the Integrated Hospital Discharge Team and Intermediate Care Bed Services, based at Warrington and Halton Hospitals.
**Main duties of the job**:
As a Senior Social Work Practitioner, you will be a valued member of the leadership team within the service to effectively support, mentor and guide colleagues in their roles. You will have a team of colleagues to directly supervise and take responsibility in mentoring and inducting new starters within the team, including Social Workers who are completing their Assessed and Supported Year of Employment (ASYE).
We are looking for experienced, dedicated and passionate Social Workers to join our team. As a Senior Practitioner, you will carry a small case-load, and support the Team Manager and Service Manager with leadership and team development. Your day-to-day responsibilities could include authorising work, allocation of cases, supervisions, legal literacy, complaint investigation, HR issues, holding complex cases, and deputising for the Team Manager. You will work closely with the Team Manager, the other Senior Practitioners, and the Transfer of Care Hub Coordinator within the team to provide an effective and well-rounded leadership approach.
Collaborative working and the ability to create and maintain effective working relationships with a range of health and social care colleagues both in the hospital and the community are an essential element of the role.
This is a part-time position, working 21.5 hours per week over 3 days on a rota basis. You will also be expected to contribute to the Intermediate Tier Service on-call rota.

The team is a multi-organisational health and social care service, with a shared vision to 'support the residents of Warrington and Halton to live well, independently at home for as long as they are able'. Our primary role is to ensure safe and timely assessment and discharges from hospital to various primary care/community settings. The post holder will be expected to work on a rota basis. The team provides a 7-day service between the hours of 8am-6pm each day. Adopting the Discharge to Assess model, you will support the team to facilitate safe and timely transfer from hospital to home or the community.
DBS clearance is required.
The ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
