**MAIN PURPOSE OF POST**:
To encourage and support adults with a learning disability and associated mental health needs to develop and lead purposeful and values lifestyles, encouraging use of local and wider community facilities, acknowledging the rights and responsibilities and achievement of personal aims and goals.

**DUTIES & RESPONSIBILITIES**

**To work in CQC rated GOOD Company**

As a support worker you will support people who have a learning disabilities, autism and associated mental health concerns with their daily routines to live the best lives possible.

Supervise the promoting and administration of prescribed medication and perform any other duties health and wellbeing of the client.

Accurately complete all records and paperwork as requested by the line manager and adhere to GDPR.

Support with all holistic educational, work related and leisure based activities.

Strong communication skills, patient, caring, empathetic, a good listener and kind.

To assist in the design and implementation of person centred care planning.

Enable service user to improve self management techniques.
To attend mandatory training, team meetings and self directed e-learning modules, in-house face to face training programme to assist with professional development.

To promote safeguarding and wellbeing for service users to high standards.
To assist and work alongside social and clinical professionals as part of a Multi Disciplinary Team.

Flexibility to support service users during evening and weekends.
Other duties as required by your manager.

You will be able to come to work every day and use the best of your personality and skills, as you support people who share interests to do the things they love most. We're passionate about developing our own people and this role can lead to other exciting opportunities within our company.

You may be requested to perform additional duties other than those stated above, appropriate to the position of Support Worker and those, which are deemed reasonable by the Registered Manager.
