South Warwickshire Foundation Trust is committed to bringing care closer to home and a vacancy has arisen for an enthusiastic Band 4 Assistant Practitioner within our Community Response Teams.
The Community Response Team operates across the hours of 08:30 to 21:30, 7 days a week, including weekends and bank holidays. Shifts are allocated pro rata, we offer a mix of early, late and long days.
We are looking for a flexible and organised individual to join our team. Whether you are newly qualified, or already an experienced Assistant Practitioner, we will support you to become an autonomous practitioner within our team.
You will be working collaboratively with the other community service teams to deliver seamless care across the locality, working within an unpredictable and unscheduled workload.

The Community Response Teams’ aim is to prevent hospital admissions and facilitate hospital discharges within a timely manner. You will also work closely with the Multidisciplinary Team, including: Therapists, GPs, Social Services, Urgent Response and many more. You will be working collaboratively with the other community service teams to deliver seamless care across the locality, working within an unpredictable and unscheduled workload. You will be responsible for managing the care of long term conditions, supporting patients at the end of their life and providing holistic, comprehensive care to patients on our caseload. Our aim is to prevent hospital admissions and facilitate hospital discharges within a timely manner. You will also work closely with the Multidisciplinary team including: Therapists, GPs, Social Services, Urgent Response and many more
