**The job itself**:
An exciting opportunity has arisen to join Wolds Community Wellbeing Team as a Care Coordinator (x 2 posts).

Under current legislation, primarily the Care Act 2014, Mental Capacity Act 2005 and Human Rights Act, you will be completing strength based assessments, care and support planning and undertaking reviews with service users in their own homes. In line with strength based approaches, a key aspect of this role is the promotion of independence, wellbeing and choice.

The role involves working with people over the age of 18, their families and carers with a key focus on supporting people to remain independent within their community settings. The role will involve accessing hospital and residential care home settings. We are looking for an individual with experience of working with adults, well developed communication skills and confidence to work collaboratively with a range of professionals to ensure the eligible needs of service users and their carers are met.

The role will give you the opportunity to develop a range of skills and knowledge, with a focus on continuous improvement in relation to quality of work and the development of an open and transparent team culture where social care practice can thrive.

Regular supervision and support will be provided, as well as an induction into the role.

Currently the teams operate Monday - Friday, but this may be extended to cover a 7 day service in the future.

The ability to travel across a wide geographical area is necessary for which a mileage allowance is payable.
