Our member practices include The Medical Centre, Bower Mount Medical Practice, The Vine Medical Practice and Brewer Street Surgeries which provide primary care to residents in the Maidstone Kent area.

**Responsibilities**:

- Management and prioritisation of a varied caseload of registered patients over the age of 18 years
- Supporting patients with housing, benefits, care needs and other queries as required
- Liaise with the wider PCN team to ensure excellent communication
- Maintain detailed records and escalate cases outside of your scope to the PCN manager as necessary

Person Specification:

- Good IT skills with experience of using Microsoft Office software would be advantageous
- **NHS Experience is preferred however we would consider applicants without any NHS experience**:

- **Experience of working in the voluntary sector, housing support, social care will also be considered**

Desirable experience:

- Education to GCSE level or equivalent professional experience
- Previous Primary Care employment or related social care experience
