A new exciting opportunity has arisen within the Medicine Care Group at Sheffield Children's NHS Foundation Trust to work as a Pathway Co-ordinator. There are 2 available roles, both are full time.

1 role is working at the Acute Site on Western Bank

1 role is working at Ryegate Children's Centre on Tapton Crescent Road

**The Medicine Care Group consists of several teams, including**:
Gastroenterology

Respiratory

Sleep

Endocrinology

Rheumatology

Oncology

Haematology

Allergy

Dermatology

Neurology

Neurodisability

You will liaise closely with the clinical teams and management teams to ensure patients receive timely and appropriate appointments according to their care pathway.

To support the effective delivery of pathways within Medicine Care group. To work in conjunction with the Care group teams to facilitate the smooth running of the patient journey through their pathway. To fully understand and provide local expertise on the national Referral to Treatment (RTT) 18 weeks target. Develop an in depth understanding of the various patient pathways associated with each specialty.

Deliver a robust system for validating and analysing data and tracking patients along the 18 week pathway. Flag up blockages and bottlenecks in clinical pathways that will affect the achievement of the target. Work with clinical, administrative, and managerial colleagues to redesign pathways to deliver smooth, fast pathways of care.

Fully understand and provide local expertise on the national Referral to Treatment (RTT) 18 weeks target.

Develop an in depth understanding of the various patient pathways associated with each specialty.
