Social Prescribing Link Co-ordinator

Chelmsford CVS is again expanding its social prescribing team and is looking for more conscientious team members with expertise in working with a range of people, some of whom may require additional support to engage with local activity.**_

We are seeking to fill, for an immediate start, one full time role of 37.5 hours per week to work with adults as part of the Centre’s Link Co-ordinator team based in South Woodham Ferrers.

The appointment offers an exciting opportunity to build on the successes of a well our team of social prescribers and their benefits to local people. Taking referrals from a range of partners including Crouch Vale Medical Centre GP practices and Multi-Disciplinary Teams within the Primary Care Network, you'll work with individuals encouraging them to connect or reconnect with support available in their local community.

You’ll be working on a one-to-one basis using a person centred approach to improve their health and wellbeing. You’ll have the full support of the CVS in connecting to the community offer and that of the Practice in supporting referrals. The project uses a holistic approach, encouraging people to build on what is strong in their lives to work towards improved wellbeing.

**Salary**:starting at £27,344 per annum rising, after satisfactory completion of probationary period, to £28,371 (pay award pending).

**Hours of work**:37.5 hours per week.

Full training and support provided during an initial induction and probationary period. Salaries are paid monthly in arrears.
30 days annual leave including Bank Holidays.
