**Join us as a Support Service Co-ordinator at Thirteen Group**:
We are looking for a Support Service Coordinator for 37 hours a week to join our young parent supported housing schemes in Durham which supports young families that need support to be able to live more independently.
**Role Overview**:
**Thirteen Group** is on the lookout for **a Support Service Co-ordinator to **join our Care and Support Team. School View and Coney Avenue both have nine flats along with a range of communal facilities, and youll be based in the office right at the heart of this scheme.

**Key Responsibilities**:
- Youll run activities linked to seasonal events or celebrations or activities that help improve social isolation and specific learning linked to support. Well give you the creative freedom to provide positive outcomes for our customers.
- You would be dealing with any referrals received and assessing suitability for the service.
- You will work with individuals to create person centred support plans and hold weekly keywork sessions to achieve their goals identified.
- You'll manage risk and safeguarding for all residents.
- You will attend multi agency meetings and provide written reports.
- Managing void properties and allocations.
- You'll be responsible for managing building compliance and health & safety of the scheme.
- Working as a team of four, youll work across a two week rota, covering our operating hours of 9am to 5pm on weekdays. Youll be required to work one in two Saturdays, and the weekend you work, you will have the Monday after off. You will be expected to be on call one in every four weeks.
- Youll also have the opportunity to pick up additional hours on a relief basis, working across other schemes to gain insight into other parts of the business and create strong working relationships across teams.
**About You**:
- Youll have experience of supporting vulnerable client groups, along with a good understanding of adult and child safeguarding and the complexities that it might bring.
- This role involves lone working at times, so youll be able to thrive in your own environment, as well as part of a team.
- The role also includes an element of supporting complex families, in their own home, on an outreach basis across County Durham and therefore, a driving licence and access to your own vehicle is essential.
- Youll be able to prioritise your workload and supportive in a non-judgmental way as youll be responsible for enabling our customers to reach their potential and for identifying their goals through individual support plans.
- Youll support our customers in a flexible and person-centred way, so you must have emotional resilience and the ability to deal constructively with challenging behaviour.
