The Active Travel Co-ordinator is a new role supporting the PCN personalised care workforce, working in three areas of the City to support the role out of the ATSP programme.

The Active Travel Care Coordinator will play a vital role in promoting active travel initiatives within the Primary Care Network (PCN). This role focuses on encouraging and supporting patients and staff in adopting active modes of transportation, such as walking, cycling, and public transport, for improved health and reduced environmental impact. The coordinator will work as part of a cross-sector ATSP project team, collaborating with colleagues in the City Council and NCVS (Nottingham City Voluntary Sector), to develop and implement active travel programs and initiatives.

The post holder will be primarily responsible for providing a central point of co-ordination across the social prescribing system for Nottingham City to maximise effectiveness of the Nottingham City Social Prescribing Link Worker network and to support the PCN Operational Managers in the 3 ATSP focus areas to facilitate and enable delivery of Level 2 ATSP support offer. The post holder will have a lead role to develop and support data management processes to feed into ATSP programme monitoring and evaluation.

Within this there will be a strong focus on leading project work and collation of practice data. It will include identifying and understanding patient cohorts, patient engagement and administrative tasks that support
the smooth and effective running of the PCN. The administrative support to the primary care network, will include activities such as organising meetings, recording minutes and action logging. You will be expected to work as a team but also using your own initiative. Ideally, we are looking for
someone who has experience with SystmOne, as well as having a good understanding of healthcare, however full training, and development, will be provided.
