There is an exciting opportunity for an experienced nurse to join our forward thinking integrated community team. The successful post holder will take an active role in the clinical leadership of the service and work in collaboration with the team leader in managing the integrated care team on a day to day basis.

In central west neighbourhood our aim is to build on the excellent care we provide to patients. We embrace ideas and all staff have an opportunity to suggest and guide change within the team.

This role will have a focus around the clinical governance of the service.

**This will include but is not exhaustive to**:

- Supporting the delivery of an integrated approach to patient care across all professions.
- Identifying and addressing areas of clinical development across the service.
- Providing training from their own area of expertise.
- Managing the performance of staff.
- Managing complex issues in the care of patients such as safeguarding risks or self neglect.
- Ensuring safe staffing levels are in place
- Supporting the wellbeing of staff.
- Deputising in the absence of the Team Leader
- Facilitating effective relationships with internal and external stakeholders such as other primary care teams and GP Practice staff.

This role would suit an individual with leadership experience and a dynamic mindset regarding how the service can best meet it's current and future objectives.

Blackpool Teaching Hospitals NHS Foundation Trust is situated on the west coast of Lancashire and offers a full range of District Hospital services and Community Health services to this population.
