**Our community practitioners work across three pathways**: Early Memory and Wellbeing Pathway, Complex Needs Pathway, and CHESS pathway. Your expertise and compassion will enable you to build strong therapeutic relationships with patients and carers/families, to help empower individuals to enhance their quality of life and explore available support.

You will work collaboratively with a multidisciplinary team of professionals, to assess the needs of our patients, support in assessment towards diagnosis of dementia, and deliver evidence based treatment and interventions as addressed in individuals care plans.

Our Early Memory Pathway supports with assessment of individuals experiencing concerns or decline in their memory and cognition, to support with establishing potential diagnosis and maximising quality of life. Our pathway support with post diagnostic interventions as well as working in partnership/ providing insight into wider support networks available to support through the patient's journey.

Our Complex Needs Pathway provides initial nurse led assessments of patients with complex dementia, often with behaviour that challenges others, co-morbid functional mental illness and often multiple physical health needs. Our team utilises a case management approach to work closely with patients, family and carers. Interventions often include interagency working.

Care Home Education Support Service (CHESS)
- Support and education into a care home setting to help assess, formulate person centred interventions to help prevent hospital admissions and use of antipsychotic medication. The practitioner will have a pivotal role within the team, and will use the Newcastle Model (2017) to provide bespoke outreach interventions for care homes, alongside delivering an education package to help promote a person’s independence and quality of life in a setting of their choice. We aim to avoid unnecessary hospital admissions and pharmacological interventions and opt for a more holistic, person-centred therapeutic approach with an array of interventions.
