An opportunity has arisen for a full time, permanent Community Practitioner to join the Adult Learning Disability Community Team in North Cumbria.

You will work as part of the MDT and across wider services and agencies to ensure the people open to our services access care and support appropriate to their need.

You will assess, plan and implement care to achieve health outcomes that are relevant for optimal health.

You will ensure the people we work with have access to services that are equitable to the non disabled population. This will involve close working with clients, carers, families, statutory agencies and third sector agencies. You will identify health need, formulate care, assess and navigate services.

We aim to attract and retain a diverse, talented and committed workforce, who are caring and compassionate, and therefore able to meet the demands of the modern NHS now and in the future. In return we can offer a dynamic working environment in which to build a career.

You will be part of a well established MDT with support across disciplines. You will provide support and learning to students, colleagues across the wider mutli agency team and participate in supervisory processes.

You will contribute effectively to multi-disciplinary clinical discussions reporting and escalating professional concerns and respond to changes to risks in a timely manner.
