Summary

We have an exciting opportunity to join our 0-25yr Special Educational Needs and Disability (SEND) service as a SEND Lead Worker on a permanent basis.

Wiltshire Council's 0-25yr SEND Service exists to improve outcomes and make a positive difference to the lives of children and young people with SEND, and their families. The Team are dedicated, passionate and enthusiastic about the quality of support they offer which responds to the needs of each individual.

The service adopts a preventative and early help approach for coordinated child, young person and family centred services which is delivered in a way which listens to and responds to the voice of children, young people and their families.

**About the role**

As a SEND Lead Worker you will join one of our dedicated teams to either coordinate and facilitate the Education, Health and Care Needs Assessment or oversee the reviewing and monitoring process of EHCPs. You will be responsible for carrying out the administrative functions within these statutory processes whilst adhering to key Statutory Deadlines.

Within the role you will remotely liaise and coordinate services with the setting/school, young person and their family, identifying when specialist involvement is required, such as social workers, educational psychologists, occupational therapists, and any other professionals. You will make the appropriate contacts and ensure effective liaison with specialists and families is met.

You will be expected to work from a locality hub at least once a week.

**About you**

We will look to you to be passionate and dedicated and be highly organised and have extensive experience of working in partnership with children, young people and their families, particularly those with special educational needs and disability.

As well as having the confidence and ability to establish a successful and trusting relationship with families/professionals you will also have excellent IT skills and be a strong team player who is solution focused with the ability to problem solve, support and challenge where necessary.

You must hold a minimum of level 4 recognised qualification and two years’ experience of working in early years, social care, health or education.
