Function Specific Responsibilities To assist the ICB in delivering its commissioning responsibilities with regards to standards as required in The National Framework for Continuing Healthcare and Funded Nursing Care (2022). The aim of the post is to ensure that all placements and/or packages of care commissioned by West Yorkshire integrated care board -Calderdale Place continue to meet the individuals care needs. The aim of the post is to assist the organisation in co-ordination and monitoring of health care funded packages on behalf of Calderdale WYICB. The post holder will be responsible for the review and monitoring of any health funded packages, inclusive of S117.

The post holder will work with other professionals, providers, individuals and their families and/or carers to ensure that packages of care are able to meet the needs of the individual, deliver safe, effective and efficient care and are in accordance with local eligibility criteria and local contracts. The Post Holder will be responsible for the Chairing of CETR's, working with a multidisciplinary panel and will at times be expected to professionally challenge MDT decision making, ensuring the individuals care needs are paramount adhering to the commissioning policy guidelines. PRINCIPAL RESPONSIBILITIES 1. To carry out the review and assessment of individuals in receipt of either continuing healthcare ,joint funded packages, inclusive of s117 2.

To work jointly with care workers (health and social) across the ICB and Local Authority area to assist in the determination of an individuals care needs in accordance with the National Framework for Continuing Care and Funded Nursing care (2022) 3. To work with care workers (health and social) in a wide variety of care settings, on occasion across a wide geographic area. 4. To collate all relevant information necessary to inform decision making about an individuals health needs, inclusive of care review, eligibility assessment and ongoing case management.

5. To work with care workers (health and social) in order to implement a care package that meets the individuals needs for NHS Continuing Care and including specialist support or equipment. 6. To review cases for the restitution of funds including the collection and collation of information, the analysis of the information and recommendations with regard to eligibility for restitution.

7. To liaise with families/carers/advocates to ensure continued involvement in assessment processes and the sharing of information with particular regard to decisions made. 8. To ensure that the care packages are monitored on behalf of the ICB and that the service that has been commissioned for that individual is being delivered as agreed, inclusive of personal health budgets (Quality and safeguarding ).

9. To identify any concerns about the care of individuals at their review and take appropriate action to advise the commissioned service on improvements required and where necessary inform the relevant agencies in accordance with local safeguarding agreements. 10. To assist operational leads in any procedures relating to contract monitoring or safeguarding vulnerable adults.

11. Liaise with other health commissioners to facilitate and co-ordinate packages of continuing health care under special circumstances. This will be done under the supervision of a Clinical lead. 12.

To assist in the gathering and co-ordinating of appropriate information for any appeals and to liaise with the ICBs designated officer. 13. To monitor, in co-operation with others, the delivery of continuing health care packages and assist in gathering appropriate information which may be requested, for continuing care monitoring and evaluation. 14.

Ensure that regular reviews of an individuals needs take place and that any care packages are meeting needs appropriately or amended to reflect changing needs of the individual. 15. Liaise with members of the ICB and Acute Trusts and Local Authority areas in order to contribute to in-service training programmes within the Primary, Secondary Care and Local Authorities. 16.

Through appraisal identify any training needs for self and/or team. To use IM&T skills to produce information and documentation within the framework of Continuing Care, Funded Nursing Care, Out of Area Placements and treatments and all other services funded through Specialist Care Services budgets. 17. To maintain the confidential nature of clinical and management information within the requirements of the Data Protection Act, Caldicott guidelines and Trust policies with regard to confidentiality etc.

18. To participate in any research or audit as required by the team and/or ICB. 19. To prioritise, manage and delegate tasks in order that competing work areas effectively meet set deadlines.

20. To advise colleagues and members of the public of the criteria and protocols regarding the variety of sensitive and complex information retained and received by Specialist Care Services.
