**JOB TITLE: Long Term Condition Coordinator**
**REPORTS TO: PCN and Quality Manager**
**HOURS: 37.5hrs per week**
**LOCATION: Organisational wide**
**JOB SUMMARY**
30% of our patients suffer from one or more long term conditions (LTC). Proactive management of these conditions is important to ensure patients maintain a good quality of life with mínimal complications. In order to achieve this, we are seeking to build a team that works with clinicians to achieve structured proactive care, building on our past experience.
The role will cover aspects of patient care in the following five long term condition areas: Diabetes, Mental Health, Respiratory, Renal and CVD. You will be responsible for the population of these clinics, ensuring you recall the relevant groups of patients identified from created searches. You will be responsible for following up on any DNA's for these designated clinics. You will also be expected to attend regular meetings with the clinical team to ensure a high standard of continued healthcare is being provided.
**KEY RESPONSIBILITIES**:
Ø Help patients to manage their needs through answering queries, making and managing appointments, and ensuring they have good quality written or verbal information to help them make choices about their care.
Ø To liaise with patients or their next of kin regarding queries and requests and ensure follow-up action is taken if the enquiry cannot be resolved immediately.
Ø Be able to create, edit and use existing searches and document templates to ensure relevant patient groups are highlighted.
Ø Use an effective recall system to ensure the necessary patients have appropriate reviews for their condition.
Ø Follow up on any patient DNA’s to ensure they do not miss out on any required appointments.
Ø Being proactive to ensure any outstanding alerts and pop ups are actioned and all relevant data captured is appropriately stored onto the clinical system.
Ø To attend administrative and clinical meetings, occasionally preparing agendas and taking minutes when required.
Duties may vary from time to time dependant on current and evolving practice needs but with consultation with the post holder.
**KEY TASKS**:
Other duties and responsibilities to be undertaken may include any or all the items in the following list:
Ø Taking messages and passing on information as appropriate.
Ø Computer data entry/data allocation and collation, processing and recording information in accordance with practice procedures.
Ø Initiating contact with and responding to requests from patients, team members and associated healthcare professionals and providers.
Ø In accordance with the practice’s Health and Safety Policy, helping keep all practice areas tidy and free from obstruction and clutter.
Ø Helping to promote and maintain a positive, supportive culture across the whole of the practice team.
Ø Any other duties as reasonably requested by the partners/management and any duties commensurate with this position.
**HEALTH AND SAFETY**
- Comply at all times with the practice health and safety policies by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System (IRS)
- Comply with the Data Protection Act (1984) and the Access to Health Records Act (1990).

- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
- In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients, families and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
- Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
