An exciting opportunity has arisen with the development of this new permanently funded band 6 post in North Ayrshire Health and Social Care Partnership. You will be an individual who will support and influence patient flow from acute sites to our long term conditions wards based at Ayrshire Central Hospital (ACH) and our community hospitals on Arran and Cumbrae ensuring patients are on the most appropriate care pathway resulting in improved patient experience/outcomes.

You will work closely with the North Ayrshire Social Work team based at University Hospital Crosshouse (UHC) and work collaboratively between UHC and ACH however will also be required to consider patient referrals from University Hospital Ayr as required. You will report directly to Clinical Nurse Manager for LTC who will actively support you with developing this role to its full potential.

You will play a pivotal role in supporting multidisciplinary ward teams and medical staff in the Acute service and Health & Social Care Partnership colleagues to ensure patients who require ongoing care in an alternative setting such as general rehabilitation, neurological rehabilitation, stroke rehabilitation, hospital based clinical complex care and end of life/palliative care are transferred appropriately in a patient centred, safe and timely manner.

You will provide evidence of ongoing professional development and be committed to actively contribute to the development of the role by reviewing and promoting patient pathways, delivering education and raising awareness of the role and developing audit to monitor effectiveness and efficiency of patient pathways.

You should possess excellent communication skills, decision making skills and have the ability to form excellent relationships with a wide range of stakeholders.
