A rare and exciting opportunity is available in NHS Ayrshire & Arran for individuals with substantial clinical and leadership skills to join the Acute Care of the Elderly (ACE) Practitioner and wider frailty team, based within in our two acute hospitals.
Ayrshire and Arran is implementing a frailty programme development across HSCPs and acute services to provide a new innovative model of care for our frail older people.

The ACE Practitioner will work in an inter-agency, inter-disciplinary team i.e. Medical Staff, Allied Health Professionals, Social Work colleagues and carers. You will work as autonomous practitioners to provide person centred, specialized comprehensive geriatric assessment, intervention and plans to care for patients with a diverse range of complex presentations and multiple co-morbidities; preventing inappropriate admission to hospital and facilitation of early supported discharge.

You should be able to demonstrate competencies in supporting individuals with frailty and complex needs. You must have excellent communication skills, the ability to adapt to changing levels of activity and acuity within the Emergency Department/receiving units and wards. Also knowledge of NHS and Social care pathways and system. Ensuring people have the opportunity to return to their own homes timeously with community-based management where appropriate and avoid unnecessary acute hospital admissions.

You require to be educated to Degree level with DIP/BSc with current Professional registration or equivalent, with relevant post-graduate registration qualification or experience in older people’s assessment/ community/ rehabilitation.

This post requires the completion of advanced clinical practice modules; therefore if clinicians do not have this level of qualification at time of recruitment the expectation is these will be commenced within first 18 months in post.
